Dear Families,
It is time! Do not miss your opportunity to provide your school with valuable feedback!

Families can take the survey online at NYCSchoolSurvey.org (in any of the nine NYCDOE supported languages) using their unique access code which is also printed on the survey.

The access code is a lowercase “f” followed by your child’s nine-digit student identification (OSIS) number. You can find your child’s OSIS number on their report card, NYSCA account or you may contact the school.

Families that have a NYSCA account can log directly into their account to do the survey. Please keep in mind that this year you will have to fill out a survey for each of your children.

Paper copies of the survey will be sent home shortly. If you need assistance with completing the survey, please contact Ms. Harris @ 347-444-6379. I will also be available at dismissal for your convenience. If this time does not work for you, call me to let me know what time is best for you!
